Counseling services are offered to adults, adolescents, children, families, and couples. It is our goal to integrate the mind, body, and spirit while assisting you in learning the truth about who you truely are and why you are dealing with your current concerns.
Counseling services are $110.00 for the initial session and $100.00 for every session thereafter.
Groups are available as needed. For example, groups for children/teens could address -- social skills, anger management issues, emotional eating, teenage wise choices. These groups discuss issues such as: peer pressure, developing and maintaining friendships, abstinence choices related to sex/drugs and developing independence.

| Getting an authorization from your insurance carrier
If you are choosing to use your insurance benefit, most insurance carriers require an "authorization number". For more information to decide if you want to use your insurance, click here. You must first call your insurance carrier to obtain this number. This is required if you want your insurance to pay for the visit. After an appointment is made, you would tell them you have an "initial visit" with Michele Kratochvil on whatever date and time. You can bring this authorization number with you to the first visit. This authorization number is "therapist specific" therefore you can not use this number to see another therapist.
Also, for your own information regarding your insurance benefits, you may want to ask the case manager:
1.How many visits do I have per calendar or plan year?
2.What is my co-pay?
3. Is marriage or family therapy a covered benefit?
4. How many visits is this authorization good for?
5. Do I have a deductible? If so, what is it?

The Informed Consent is for your information. You only need the last page on the Informed Consent for your file. Your signature on this page indicates that you have read and received this information. There is also a space for an emergency contact. Please complete this area also.
Please bring the paperwork with you to the first session. If you choose not to fill out the paperwork prior to our first session, please arrive 15 -20 minutes earlier than your scheduled time, so you will have ample opportunity to complete the forms.
